GEMRU-RECORD-REQUEST-FORM

Per HIPAA and FERPA standards, patients must fill out this form and follow the included instructions to obtain a copy of their records from GEMRU. All of our records are stored and managed securely following HIPAA standards.

To obtain a copy of their GEMRU patient records, the patient must visit the UF Public Safety Building in person with a valid form of ID to verify their identity. Once there, they must complete a records request form. If the request is valid, we can release the patient care report.

The only other circumstance in which we can release a patient care report is if compelled by a subpoena, as HIPAA regulations protect these records.
